The Unity Home Care Plan provides life insurance cover on the life of the secondary lives or the primary life (policy owner) and pays the selected benefit in the eventuality of death.

TERMS & CONDITIONS
  • Home CarE PLAN
    Home Care Plan is a group term assurance policy providing funeral finance assurance to customers of Unity Link as well as their relatives.

  • Insurability
    Every healthy person of at least eighteen (18) years of age, sender of remittance through the platform of Unity Link and less than Seventy Five (75) years of age at the moment of expiry of the insurance, is insurable.

  • Incontestability
    The declaration of the insured member is the basis on which the corresponding individual insurance certificate is generated. This certificate becomes incontestable starting from its effective date, except in case of an intentional false declaration, an omission or an inaccuracy in the declaration of the risk evaluation elements. This certificate is delivered to Unity Link, who should in turn deliver it to the insured member.

  • Effective Date of Membership
    The effective date of the insurance is the inception date of the contract between Unity Link and Vanguard Life or the date the number of remittances required granted by Unity Link, whichever is latest.

  • Miscellaneous
    An insurance certificate is issued by Vanguard Life. The certificate and its corresponding contract shall in all respects be constructed according to the laws of Ghana, and no liability shall attach to the Company under any judgment or decree, save a court in Ghana.

  • Premium
    The premium payable is a single premium which is paid in advance at inception. The premium may be amended by Vanguard Life, if necessary. This revision shall be based on actuarial experience.

  • Definition of Life Cover
    The maximum life benefit provided under the Policy is the benefit level subject to a maximum of GH¢10,000. This cover is subject to a waiting period of three (3) months from the commencement date of the policy.

  • Definition Of Disability Cover (Total Permanent Disability)
    In case of total and permanent disability due to accident or illness, the payable benefit equals the selected benefit level at the date of Permanent and Total Disability. Deferred period of three (3) months from the date of incapacity.Consequently, the benefit under the policy shall not be payable if disability occurs within the three (3) months waiting period.

  • Claims Procedure
    a. The beneficiary would be paid the guaranteed amount (benefit level selected) in case of death of the insured member and or the insured family member within seventy – two [72] hours.
    b. Proof of enrollment as Home Care Plan member (i.e. the certificate.)
    c. Original or authenticated Medical Certificate of Cause of Death or letter from local chief or religious leader (e.g. pastor or imam of a recognized church or mosque).
    d. Police report in the event of accidental death.
    e. National photo identity card (ID) of the claimant.

1. Notify Us

2. Claims Procedure
